Dear parents / carers,
This year, the date for the flu nasal vaccination is Friday 1st December 2023. For your child to be given the flu vaccination in school you will need to fill out an online consent form.
The form can be accessed at www.leicsandrutlandimms.co.uk/Forms/Flu
You will need a code for your child’s school. For Bishop Ellis Catholic Voluntary Academy
Thurmaston the code is LE120218
Select Seasonal Flu and enter the school code. On the consent declaration page, you can consent for either the nasal flu vaccine or the injectable flu vaccine.
Top tip: Ensure that the email addresses match and that there are no spaces or punctuation before or after the email address and the school code.
If you are unable to complete this online, please ring Community Immunisation Service to complete this over the phone on 0300 300 0007
(Select option 1 then option 4).
The online form will make the process easy and efficient and will avoid letters being lost in transit. The form meets all of the NHS security standards for patient information.
If you’re having problems submitting the form, it’s also a good idea to double check that when you confirm your email address, it matches the one originally submitted.
